We provide residential homes to support adults transitioning from hospital settings into more independent community life. Our homes offer a short-term, structured environment that balances safety, stability and personal growth.

Our Transition Homes

We offer four residential properties in Bristol, each thoughtfully designed to support people’s recovery journey:

  • Three-story, three-bedroom houses with welcoming, home-like environments
  • Single occupancy living, with flexibility for shared arrangements based on compatibility and support needs
  • Adults aged 18 and over, with tailored support to match individual pathways

Each home provides a calm, therapeutic setting where people can focus on their next steps.

A Short-Term, Focused Stay

Residency is offered on a 0-9-month contract. This ensures the focus remains on progressive, person-led transition rather than long-term placements.

Integrated Professional Support

Every person staying in our transition homes benefits from access to a dedicated network of health and care professionals.

  • Community Psychiatric Nurses (CPNs) play a key role in helping people transition smoothly from hospital to home. They offer mental health support, assist with medication management, and provide emotional guidance. By building strong, trusting relationships, CPNs help people adjust to community living while focusing on their lon-term well-being and recovery.
  • Positive Behaviour Support (PBS) specialists work closely with each person to understand their experiences and develop proactive strategies that promote positive outcomes.
  • Our Therapy Team contributes additional therapeutic input, tailoring the support to each person’s specific needs and goals throughout their transition journey.

Our homes are not charity-run, NHS-managed, or hostel-style facilities.

They are residential properties fully owned and operated by Nurseline Community Services, designed to provide respectful, dignified living experiences aligned with each person’s individual goals.

Support is delivered by our experienced multidisciplinary teams, blending therapeutic input with practical day-to-day assistance tailored to each person’s unique needs.
