We have developed an assessment to support people facing delayed discharge from hospitals or out-of-area placements. It is carefully developed by our internal team of Community Psychiatric Nurses (CPNs).
Our goal is to assess the risks the person faces, recommend pathways for their return to suitable accommodation, and explore engagement with private housing providers.
