We use multimedia tools such as video avatars and social stories to help people understand, choose and embrace their journey forward.

Led by Community Psychiatric Nurses and Multidisciplinary Team

At the heart of every successful transition is a team that works together.

Our services are led by Community Psychiatric Nurses (CPNs) who play a key role in clinical oversight, mental health assessment, and providing proper support. Working alongside our CPNs is a multidisciplinary team, including:
