Our multidisciplinary team delivers person-centred support, helping people stay connected to their communities while receiving the required care.

Bridging the Gap Between Hospital and Home

Our Community Psychiatric Nurses (CPNs) provide personalised, in-home support to help people transition smoothly from a hospital to a community setting. By addressing both clinical and practical needs, our CPNs ensure that each person feels safe, supported, and empowered in their new environment.

Comprehensive Mental Health Support

• Assessment tailored to individual needs 
• Therapeutic interventions for emotional well-being and stability 
• Crisis intervention, including home treatment for urgent support

Ongoing Monitoring and Person-Centred Care

  • Risk management and proactive assessment of changing needs

Our Therapy Team combines three crucial fields of support: Positive Behaviour Support, Occupational Therapy.

Our team provides evidence-based behavioural assessment to create tailored therapeutic plans based on the person’s capabilities.

Assess

Identify the person’s physical, psychological, and emotional needs.

Plan

Develop a personalised care plan based on the assessment and align care goals with the person’s preferences.

Implement

Deliver the planned care with guidance from the multidisciplinary team.

Evaluate

Continuously monitor the plan’s effectiveness and adapt to meet the person’s evolving needs.
