Step-Down Transition Homes

Operating as part of our Rapid Stabilisation & Community Transition Pathway

Our Rapid Stabilisation service offers a short-term, supported living option for people whose current arrangements are no longer stable or safe. It provides an alternative to hospital admission by focusing on stabilisation, recovery, and clinical care, while longer-term support plans are developed.

Care is delivered through supported homes, with therapeutic and multidisciplinary input designed to reduce escalation, maintain safety, and support people to move forward without unnecessary readmission or restrictive interventions.

Who We Support

Autistic people, people with a learning disability and/or mental health needs who:

  • are ready for hospital discharge but have limited community support options
  • are at risk of their current support breaking down
  • have limited suitable alternatives available locally
  • are at risk of readmission without the right support in place

We work closely with families, care professionals and local authorities to ensure the best possible outcomes.

How Rapid Stabilisation Works

Each person is supported through a personalised circle of support, providing safety, stability, and the right level of care from day one. The approach is shaped by individual circumstances, experiences, strengths, and aspirations, with a clear, person-centred support plan put in place to guide care.

Our Transition Homes

Our transition homes offer a short-term, structured environment that balances safety, stability and personal growth.

We offer four transition properties in Bristol, each thoughtfully designed to support people’s recovery journey:

  • Three-story, three-bedroom houses with welcoming, home-like environments
  • Single occupancy living, with flexibility for shared arrangements based on compatibility and support needs
  • Adults aged 18 and over, with tailored support to match individual pathways

Each home provides a calm, therapeutic setting where people can focus on their next steps.

A Short-Term, Focused Stay

Residency is offered on a 0-9-month contract. This ensures the focus remains on progressive, person-led transition rather than long-term placements.

During their time with us, people receive structured support that encourages:

  • Building daily living skills
  • Strengthening emotional well-being
  • Planning for future housing and community connections
  • Restoring confidence after a hospital stay
